RainbowECHO Brief Psychotherapeutic Interventions for the Primary Care Provider Course

By registering, you confirm your acknowledgement and consent to participate as a community partner with Project ECHO: RainbowECHO Community partners agree to:

• Participate collegially in regularly scheduled ECHO conferences by presenting cases, providing comments, and asking questions;
• Keep confidential any clinical information provided by other community partners during a conference;
• Complete periodic surveys to help improve services to clinicians and other partners;
  Use required software including, but not limited to Zoom and REDCap;
• Provide clinical updates and de-identified outcome data on discussed cases as needed;
  Be solely responsible for the treatment of their patients and understand that all clinical decisions rest with them regardless of recommendations provided by other Project ECHO: RainbowECHO participants;
  Ensure that their patients are aware of their participation in Project ECHO: RainbowECHO and their de-identified information could be shared; and
• Be photographed and recorded during Project ECHO: RainbowECHO sessions.

I understand that:

• Project ECHO® case consultations do not create or otherwise establish a provider-patient relationship between any UH clinician and any patient whose case is being presented in a Project ECHO® setting.
• Project ECHO® collects registration, participation, questions/answers, chat comments, and poll responses for some ECHO programs. Your individual data will be kept confidential. These data may be used for reports, maps, communications, surveys, quality assurance, evaluation, research, and to inform new initiatives.
• My contact information will be shared with other members of this program's cohort.
